Employment details

  • Full-Time Salary, Exempt, 40 hrs/week
  • Starting Salary: $65,000 plus benefits
  • Reports to: Spokane Site Manager

The Wellness Navigator will encourage social, physical, and mental well-being for our wider Pasifika communities by providing support in navigating systems and accessing services. The Wellness Navigator provides case management for Pasifika families by addressing the social determinants of health: housing security, access to transportation, food, healthcare and increase their sense of belonging in their communities.

Responsibilities & Duties

Wellness Navigation

  • Enroll 30-35 families at a time.
  • Meet with families weekly to monthly, dependent on their needs, to support in navigating services.
  • Create and complete case management files on each family enrolled – keep maintenance of files which include case notes, records, recordkeeping, service plan, etc.
  • Develop service plans for families that address the social determinants of health, in order to achieve wellness within 3 months of enrollment.
  • Provide linguistically and culturally appropriate support for Pasifika families in one-on-one meetings.
  • Provide COFA/Insurance healthcare navigation to enroll uninsured Pasifika families and connect families to Primary Care of choice.
  • Provide housing navigation for families without homes or at risk of experiencing homelessness.
  • Provide excellent community referrals that are culturally relevant for Pasifika families.

Other Duties and Expectations

  • Collaborate with other staff to ensure organizational goals are met.
  • Support in building and maintaining positive relationships with key community and organizational partners.
  • Represent PICA-WA at community events when needed.
  • Schedule flexibility is required for occasional meetings outside business hours.

Desirable Work Experience Qualities

  • Experience in and/or working with diverse Pasifika communities which include Micronesian, Polynesian and Melanesian communities.
  • Experience supporting and working with people navigating trauma and/or in crisis.
  • Strong racial and social justice analysis is rooted in Pasifika values and principles.
  • Strong written, verbal, and interpersonal communications skills.
  • Fluency in Chuukese or another Native Hawaiian/Pacific Islander (NH/PI) language, is preferred.
  • Ability to meet deadlines.
  • Commitment to accountability for improving the community PICA aims to serve.
  • Excellent organizational skills, such as convening meetings, scheduling, and note taking.
  • Experience with project management, event planning, and event coordination.
  • Knowledge of working with social justice issues and cultural framework.
  • Proficiency with Microsoft Office

Educational/Experience Requirements

  • Bachelor's degree in social work, sociology, public administration, public health, or a related field,
    or equivalent successful workplace experience.
