Pasifika Family Wellness

Our Pasifika Family Wellness programs is modeled after the Community Health Workers model in understanding that the health of our Pasifika communities is linked not just to accessing healthcare but also in addressing the social determinants of health which includes access to housing, transportation, food, social belonging, employment & education. Our Wellness Navigators are equipped to support Pasifika families in navigating services offered in our NHPI networks and offered outside of our networks. The provide supports to combat housing insecurity, access to healthcare and other critical supports. Pasifika Family Wellness also focuses on the cultural stewardship of our elders through providing elder social support services.

 

Family Case Management

  • 3 to 6 months of client family support
  • Once a week to once a month meeting as needed
  • Support clients in navigating services to address social determinants of health

Woven with Elders

  • Support programming for elders 60+ once a week at the PICA-WA Office once a week
  • Conduct wellness and health activities to support the wellness of our elders
  • activities and meal for elders for 4 hours of programming once a week on Fridays
  • Provide light case management with elders enrolled in program to assess for needs

Housing Services

  • Enroll Pasifika families in Coordinated entry
  • Provide housing support services¬†
  • Eviction Prevention & Housing Stabilization¬†

Pacific Islander Covid 19 Surveillance Program
